The FSC must notify the LEA of a child turning three when the child is 2 years, 2 months old through 2 years, 9 months of age in order to meet the timelines for Part B eligibility determination and IEP development.

Step 1

Step 1: Transition Notification must be completed with the necessary child’s information and sent to the child’s parent. Parental consent is not required to send a copy of this letter to the LEA; however, other early intervention records provided to the LEA must have parental consent for release. A copy must be sent to the LEA and the Regional OCDD office. The FSC must keep a copy in the child’s record.

Step 2

Step 2: The Transition Notification letter is sent to the family, a copy of the Early Steps Family Survey must be sent. The FSC is responsible for sending the Early Steps Family Survey along with the Early Steps Family Survey Cover Letter, which explains the process that the family must follow in order to complete the survey. The Early Steps Family Survey assists the state in measuring performance on Indicator #4 of the SPP.

Step 3

Step 3: Send out a Team Meeting Notification: FSC’s must provide a Team Meeting Notification to all Transition team members 10 calendar days prior to the meeting. This advance notification assists with scheduling for the LEA and the completion of necessary activities within the established timelines. The FSC must complete a Team Meeting Notification and send to all Transitional team members. The FSC must maintain a copy in the child record.
